Ready to take the next step towards quality care?

1. Apply through DSPD. You can do this through MySteps online, filling out paper intake forms and mailing to DSPD, emailing your intake forms to dspdintake@utah.gov, or calling 1-844-275-3773. 90 days is the maximum amount of time to complete and submit these forms before your account is listed as inactive. You will still have the option to continue if this happens, but please make sure to update any information that may have changed so we can find a caregiver that’s the right fit for you. 

2. After submitting your information, you will soon receive a Notice of Agency Action and Hearing Rights form in the mail. This will inform you whether or not you’re eligible for services along with your hearing rights, should you disagree with DSPD’s decision. You will then be placed on a waiting list for services. DSPD usually is unable to offer immediate services due to lack of funding.

3. When you’re taken off of the waiting list, a DSPD intake worker will notify you and discuss the current service options available to you. You will now have the option to request and receive services through a private provider, such as us.
